Community Case Management in Toledo

Our Community Case Management team serves youth ages 11- 19 who have survived the realities of sex-trafficking, are actively being exploited, or are at risk of victimization. Our services are based on a comprehensive, trauma-informed care model. This personalized process is tailored to meet the complex needs of the most vulnerable youth in our community, young people who have been taken advantage of, coerced, and forced into exploitation.

Meet Our Case Managers:

  • Jen Wakefield

    Born and raised in Oregon (Toledo), Ohio, Jen is a decorated Navy Veteran, serving as a combat air controller during Operation Iraqi Freedom and Operation Enduring Freedom. Currently serving in the role of Community Case Manager in Toledo, Jen has been in the nonprofit sector for the past 10 years working in many roles from education, to lobbying, and victim services. She loves working at Gracehaven because she is able to connect with and mentor youth while serving the community. Prior to working at Gracehaven Jen worked in marketing and social media for nonprofits as well as being the Director of Suicide Prevention for Lucas County which she still is involved with. Currently she is pursuing her Masters in Mental Health at Bowling Green State University.

    Born in Toledo but raised in the mountains of Northeastern Pennsylvania, Brooke has a unique expertise working with victims of trauma. From her time as an Equine Specialist working with children who experienced abuse and neglect to working with veterans and first responders with PTSD, Brooke has had many experiences and trainings to lead her to work at Gracehaven. Brooke is a certified Equine Specialist with EAGALA (Equine Assisted Growth and Learning Association) as well as trained in Equine Assisted EMDR (Eye Movement Desensitization and Reprocessing). Brooke has also been a long-time volunteer for Youth for Christ in Pennsylvania. Brooke is currently a community case manager in Toledo and is excited to utilize her skills, trainings, and passions to help those at risk or currently being sex trafficked.
